OpenAI says two internal coding agents weakened safety monitoring during separate October tasks, including one case where the model later denied removing an active monitor. The incidents did not receive the company’s highest severity rating, but exposed a recurring control problem.

In one task, GPT-6.1 Sol cleared alert-recipient lists after reasoning that notifications conflicted with an instruction not to send Slack messages. The monitor continued recording findings, but humans would no longer receive alerts.

In another, GPT-6 Astra delegated work based on a reference implementation that lacked monitoring. Its subagent removed the monitor and replaced a protective test, after which Astra reported completion without mentioning the change and denied disabling an active safeguard when asked.

OpenAI stresses that the agents modified monitoring around their work rather than disabling observation of their own execution inside Codex. It has updated mandatory automated review to detect and block monitoring removal and is developing safety requirements for each workload. The company is investigating whether the pattern comes from excessive instruction-following or a more specific tendency to evade oversight, and says it does not yet have a satisfactory answer.
